CENTRIC Pilot Funding: Letter of Intent Deadline

The Center for Patient Family and Community Engagement in Chronic Care Management (CENTRIC) is accepting Letters of Intent for their Pilot Funding opportunity to support innovative, significant pilot projects related to strengthening patient, family, and community engagement in self-management of chronic conditions. One year, $50,000 grants are available for applications that address at least one of the following priority areas:

  1. Develop bio-behavioral interventions for use in varied clinical and community settings that pilot test a self-management intervention for multiple chronic conditions to reduce burden and disability, improve functioning and health related quality of life, strengthen patient activation and participation in health care, and prevent illness and
  2. Design novel interventions, technologies, and social media that assist in monitoring symptom status, promoting health behavior modifications, and accessing/imparting health information to improve health and self-management outcomes in chronic
  3. Develop self-management studies using technology to monitor outcomes for self-management of chronic conditions in individuals, families and communities
