CENTRIC Presentation

The Center for Patient, Family, and Community Engagement in Chronic Care Management (CENTRIC) will host Patrik Johansson, MD, MPH, and Paula Schulz, PhD, RN, on Friday, December 11^th from noon to 1pm. “Cardiovascular Disease Self-Management in Rural Adults: A Pilot Intervention Study Using Technology and RN/Community Health Worker Teams” will outline the implementation of a cardiovascular self-management intervention in rural settings, provide details on the use of technology in this study, and discuss participant recruitment and data collection strategies.
