CENTRIC Pilot Grant Funding Opportunity LOI Deadline

The Center for Patient, Family, and Community Engagement in Chronic Care Management (CENTRIC) has announced the scheduled dates for LOIs and applications for their Pilot Grant. Qualified applications could receive $50,000 for one year projects. Competitive applications will address at least one of these priorities:

• Develop bio-behavioral interventions for use in varied clinical and community settings that pilot test a self-management intervention for multiple chronic conditions to reduce burden and disability, improve functioning and health related quality of life, strengthen patient activation and participation in health care, and prevent illness and
• Design novel interventions, technologies, and social media that assist in monitoring symptom status, promoting health behavior modifications, and accessing/imparting health information to improve health and self-management outcomes in chronic
• Develop self-management studies using technology to monitor outcomes for self-management of chronic conditions in individuals, families and communities.
